ADMISSION REQUIREMENTS AND CHECKLIST

THE FOLLOWING CHECKLIST WILL ENSURE YOUR APPLICATION IS COMPLETE. SUBMISSION DEADLINE FOR THE APPLICATION AND REQUIREMENTS IS SEPTEMBER 1^ST.

1. Send to DMH, the completed application including:
• Two (2) copies of your resume (noting all employment and educational experiences)
• Two (2) copies of a statement describing your purpose for pursuing a graduate level education in nurse anesthesia
2. A check for $40.00 payable to Bradley University sent to DMH along with the application.
3. Send an original transcript from all colleges, universities, and nursing schools directly to both DMH and Bradley University documenting the following:
• Completion of an NLNAC or CCNE accredited BSN program.
• Undergraduate GPA of 3.0 (on a 4.0 scale)
• Undergraduate health assessment course with a grade of "C" or better
• Undergraduate nursing research course with a grade of "C" or better
• Statistics course (inferential statistics) with a grade of "C" or better
• Inorganic chemistry course with a grade of "C" or better
• Organic chemistry course with a grade of "C" or better
4. Three letters of recommendation sent to DMH:
• One from a BSN faculty member
• One from your current ICU nursing supervisor
• One from a professional nursing associate
5. Have either the GRE or MAT sent to both DMH and Bradley University
   • GRE (verbal and quantitative)combined score of 1000 for an unconditional acceptance
   • MAT score of 391 for an unconditional acceptance
6. Send two copies of Illinois registered nurse licensure to DMH. Must be license eligible as a registered nurse in Illinois
7. Send two copies to DMH:
• Current ACLS (Advanced Cardiac Life Support) certification
• Current BCLS (Basic Cardiac Life Support) or CPR certification
• Current PALS (Pediatric Advanced Life Support) certification
8. (foreign applicants only) have completed the Test of English as a Foreign Language (TOEFL) with a score of 500 sent to Bradley University
9. Minimum of one (1) year adult ICU experience
• Current and ongoing ICU is expected
10. Completed Assessment of Critical Care Skills Form submitted to DMH.
