Applying For Membership
To take advantage of the benefits offered with a PrimeTime membership, print the form out and send it with your one-time $10 membership fee for DMH PrimeTime, 2300 N. Edward St., Decatur, Illinois, 62526. Please allow two weeks for receipt of your membership card.
