Oncology Social Work
DMH Cancer Care Institute provides a licensed social worker to support and assist individuals with cancer, caregivers, and families. Services include supportive counseling; emotional support; and help with practical concerns, such as advanced directives, community resources, financial assistance, and more. The DMH Cancer Care Institute Survivorship Program helps cancer patients and their families move beyond treatment. This program facilitates a positive transition from the end of active cancer care to optimal wellness after treatment.
Patients receive a personalized treatment summary and care plan (approved by the patient’s treatment team) including possible late-and long term effects of treatment as well as health maintenance recommended by the National Comprehensive Cancer Network (NCCN).
