As rehabilitation experts, we developed the DMH Bridge Program to provide a “bridge” into a community-based or independent exercise program. The DMH Bridge Program is coordinated by a certified athletic trainer in consultation with occupational/physical therapists and provides a safe exercise regimen in a medically supervised facility.
The Bridge Program is designed for:
- Patients who have been discharged from a formal rehabilitation program and wish to continue exercising under the supervision of healthcare professionals;
- Patients whose insurance prohibits further rehabilitation visits but who still require supervision from a health care professional in a clinical facility;
- Patients whose doctor requests medically supervised exercise but do not meet the criteria of formal rehabilitation;
- Patients who are scheduled to have or have had a total joint replacement or reconstructive surgery and wish to use therapeutic rehabilitation pools.
